Baldur’s Gate 3 is filled with hidden secrets, and one of them involves rescuing the Kuo-Toa, a reclusive fish-like race, from the clutches of Boooal, their alleged deity. This encounter occurs in Act I, but it’s easy to miss if you’re not vigilant. Here’s a guide on how to save the Kuo-Toa from Boooal and make the right choices during this event.

Locating the Festering Cove
To find the Festering Cove and rescue the Kuo-Toa, you’ll need to venture into the treacherous Underdark, a subterranean world beneath the Sword Coast. There are multiple ways to access the Underdark: you can use Feather Fall down a chasm from the well in the Blighted Village, ride the Zhentarim elevator from the Zhentarim hideout, or explore below the Goblin Camp to reach the abandoned Temple of Selune. Be cautious, though, as the Underdark is a perilous place.

As you explore the Underdark, watch out for Torchstalk, a common orange mushroom that explodes into a fireball if approached too closely. Shoot it from a distance with an arrow to safely destroy it. Head to the southern part of the Underdark map, situated between the Arcane Tower and the Temple of Selune. You’ll discover a gap with mushrooms leading to a ledge adorned with Torchstalk. Eliminate the Torchstalk, jump your party across the mushrooms to the ledge, and descend the Cragged Wall to enter the Festering Cove.
Rescuing the Kuo-Toa
As you approach the Kuo-Toa, your character must pass a Nature check to recognize them and their unusual deity, Boooal, who bears a suspicious resemblance to Bhaal, the god of murder. Choose a character with a high Wisdom stat for a better chance of success. Tell the Kuo-Toa, “You can give it a rest. Your ‘god’ isn’t real.” Next, you’ll need to roll and succeed in a Persuasion, Strength, or Intelligence check—choose the stat that best suits your character. Finally, inform Boooal, “I’ll just kill you and claim it for myself.”

Now, you’ll confront Boooal in a not-so-difficult battle. Be cautious not to accidentally provoke the Kuo-Toa with area-of-effect attacks.
If you fail these skill checks, both the Kuo-Toa and Boooal will attack you, and you’ll have to defeat them both, yielding no benefits. The last resort is to obey Boooal, resulting in the death of one of your companions or an agreement to kill for Boooal. A companion killed this way cannot be resurrected, even with a Resurrection Scroll. However, killing your friend grants you the permanent buff, Boooal’s Benediction, which also comes with Boooal’s Sickle.
Choosing a Deity for the Kuo-Toa

The Kuo-Toa need a new god to worship, and you have several choices. You can introduce them to a different deity or have them worship you. You can also let them go on their way or continue worshipping you. Your choice here doesn’t have a significant narrative impact, so consider what aligns with your character’s beliefs.
As a reward, you’ll receive Boooal’s Sickle, a rare weapon that grants the wielder Boooal’s Benediction, providing an advantage when attacking Bleeding Targets.
